Introduction

The Adult ADHD Self-Report Scale (ASRS-v1.1) Symptom Checklist was developed in conjunction with the World Health Organization (WHO), and the Workgroup on Adult ADHD that included the following team of psychiatrists and researchers. This scale is designed to assess the frequency of ADHD symptoms in adults based on DSM-IV criteria. Please answer the questions below, rating each item according to how you have felt and conducted yourself over the past 6 months.

For each question, please select the response that best describes how you have felt and conducted yourself over the past 6 months.

Scoring Method

  • Each question is scored from 0 to 4 points (Never=0, Rarely=1, Sometimes=2, Often=3, Very Often=4).
  • Part A (Questions 1-6): Screening questions, 4 or more positive responses suggest ADHD symptoms.
  • Part B (Questions 7-18): Additional symptom assessment.
  • Total score range: 0-72 points.
  • Higher scores indicate more severe ADHD symptoms.

Dimension Explanation

  1. Inattention:Difficulty sustaining attention, following instructions, and organizing tasks.
  2. Hyperactivity:Excessive motor activity, restlessness, and difficulty sitting still.
  3. Impulsivity:Acting without thinking, interrupting others, and difficulty waiting.

Notes

  1. This scale is a screening tool and does not provide a diagnosis of ADHD.
  2. If you score high on this scale, consider consulting with a healthcare professional for proper evaluation.
  3. ADHD symptoms must be present before age 7 and cause impairment in multiple settings for a diagnosis.
